Please do not over write, scratch or soil the application. Write only one letter per box. Leave one box as a gap between words. Never write too big or too small letters in boxes. Many fields on the application are self explanatory. Further guidelines are given below for information:
Test Name: It should be filled by candidate on Main Application, on Examination Hall Entry Pass (Hall Ticket) and on Invigilator’s Reference. For example: if you want to apply for fellowship for Polytechnic course during the year 2008, you should write PL108 in this field on application form.
Candidate’s ID Number: Please leave this field blank on Main Application form, on Examination Hall Entry Pass and on Invigilator’s Reference. Universal Fellowships will fill this information for you.
Year:Please write current year (2008) in this field on Main Application form, on Examination Hall Entry Pass and on Invigilator’s Reference.

Please note that Universal Fellowships can not guarantee availability of all the centers mentioned in the list for the tests. Factors such as number of students appearing, facilities, etc. will influence the availability of a center for a test. Generally, 1st choice of a candidate will be considered for his/her examination center. If candidate’s choice of centers are not available, Universal Fellowships make final decision to allot nearest possible center to that candidate to appear for the test.
Medium of Examination Preferred by Candidate: PL108, EN108, MM108 and ED108 tests will be conducted both in English as well as in Telugu medium. Candidate should write only one medium on the application form, on Examination Hall Entry Pass (Hall Ticket) and Invigilator’s Reference.
Phone Number of the candidate: Candidate may write his/her contact phone number with STD code in this field (Example: 08662584690). Leave it blank if phone number is not there.
Email of the Candidate: Candidate may fill his/her email address in this field. Leave it blank if email is not there.
Examination Fee Details: Fill this field after you obtain a crossed demand draft/ Indian Postal Order of Rs. 100/- drawn in favor of CEO, Universal Fellowships, Poranki, Vijayawada from any nationalized bank or post office, respectively, payable at Vijayawada. Do not pay fee in cash. And, do not pay cash to any representatives or third persons. Universal Fellowships will not take any responsibility for any cash paid to anybody. Examination fee is not refundable and applications without prescribed examination fee will not be accepted.
Candidate’s Name in Full: Apart from main application form, candidate should not forget to write his/her full name on the Examination Hall Entry Pass and Invigilator’s Reference.
Date & Center of Examination: Leave this field blank on Examination Hall Entry Pass and on Invigilator’s Reference. Universal Fellowships will fill this information and send Examination Hall Entry Pass to the candidate.
Passport Photo: Affix not more than 3.5 x 4.5 cms size passport photos on Main Application, on Examination Hall Entry Pass and on Invigilator’s Reference. Do not forget to sign on these three passport photos.
Signatures: Candidate and his/her father/guardian should sign the application in the respective boxes shown on the application. Applications without these two signatures will not be accepted.
Attestation with Seal: Completely filled application form should be attested with seal by Head Master/ Principal/ any Central or State Government Employee/ Bank Employee or any Gazetted Officer. Applications without attestation or seal will not be accepted.
Name and Address of Candidate’s School/Institute: Write your current school/institute name, address and PIN code. If you are not currently going to any school/institute, please write ‘Not Applicable.’
Candidate must fill the required fields on Examination Hall Entry Pass and Invigilator’s Reference. Affix stamps worth Rs. 5/- in the specified location on Examination Hall Entry Pass.
